Thames Reach runs projects and services that help to end homelessness in London. We work directly with homeless people on the streets and in hostels. We work to prevent vulnerable people from becoming homeless. We help formerly homeless people get their lives back on track. We help people find work and access training and volunteering opportunities.

About this role…

Floating Support services work with people to sustain their housing. We use a strength-based approach to work with people to identify what support they require and how we can address them together or / and in collaboration with others e.g. landlords, Department of Work and pension or mental health services.

We can support people for the duration of their tenancy, or we can provide short term interventions. We can provide resettlement support when a tenancy commences, and we can equip people with the skills to manage their home and goals independently or when they are move on ready and require minimal support.

Thames Reach welcomes applications from people with transferable skills.

You will:

* Visit clients in their own homes and across the community, offering 1-1 tenancy-related support

* Deliver support to a caseload of clients with complex and multiple support needs

* Use a trauma-informed and co-produced support approach

* Work with the people we support to create and review Risk Assessments and support plans in line with Hounslow Reach service timescales

* Work in partnership with Hounslow Reach colleagues and other services, including health, housing, social care and Employment & Skills teams, to deliver holistic, co-ordinated support to clients

* Contribute towards service evaluation and improvement through engagement with the people we support, with colleagues, and with other key stakeholders.

To succeed as a Support Worker, you will:

* Work to manage, mitigate and reduce risk to clients caused by health, safeguarding or welfare concerns

* Be open to learning and using Trauma-Informed and Strengths-Based approaches to motivate and enable the people we support to achieve their goals and develop their resilience

* Be reflective, flexible and responsive in meeting the needs of the people we support

* Be confident in advocating for the people we support through joint work with partner services

* To promote, and positively represent Thames Resch in meetings and events with external services

* Be organised and manage your time to meet the needs of the people we support.
