A large NHS organisation is looking to appoint a Clinical Case Manager, this role will oversee CAMHS caseload only.
This role would be suitable to a senior Health or Social Care professional who has experience working within CAMHS services, case management experience is not essential as can be trained within this quite quickly.

Key Responsibilities will include:

As a Case Manager the post holder will work as part of a dynamic team in delivering an effective service to ensure safe and effective care pathways are in place for all service users in the West Midlands Region specialised mental health, learning disability and autism care settings, within the NHS and Independent sectors;
Will provide clinical support for a designated caseload to enable the ongoing therapeutic interventions of service users in specialised secure mental health care;
The aim of this clinical support is to have oversight of robust access assessments, ensure that each service user is placed in the least restrictive setting with the appropriate level of supportive interventions to enable their eventual discharge into the community where this is a safe option or maintain their individual needs if they remain in a secure care setting;
The role is designed to use a combination of subject matter expertise and technical skills to project mapping of future service use, identifying and addressing pathway barriers that prevent the repatriation of service users out of area and identifying and addressing challenges that prevent timely discharge of service users from a secure environment;
Provide clinical support to the specialist commissioners in relation to the defined population, this will include projected mapping of future service use, identification of users for future service provision, planning in terms of future service needs.
The ideal candidate will have:

Registered Mental Health/Learning Disability Nurse/Allied Health Professional or other registered health or social care professional;
Extensive knowledge and work based experience within CAMHS settings, this can be secure or non secure settings;
Must have an understanding of the background to and aims of current healthcare policy and appreciate the implications of this on engagement;
Due to the nature of this work, candidates will need a valid driving licence and access to a vehicle to undertake the role.
