Job Description
Decentralized and centralized pharmacist activities supporting the 425-licensed bed hospital.  Activities may include supporting the Emergency Department which averages 180+ visits per day and hospitalist provider for ED admissions, the observation unit, the 24-bed Intensive Care Unit, Cardiovascular Intensive Care Unit or general medicine units with some centralized activities.    
Decentralized activities approximate 60% of time.

Under minimal supervision, functions in all sections of the Pharmacy; prepares drugs for distribution; provides drug information and clinical pharmacy services; maintains drug control; provides work direction to certified and non-certified Pharmacy Technicians.
REQUIREMENTS
Reviews all medication orders verifying the completeness and appropriateness of drug therapy and obtaining clarification of questionable orders as necessary before preparing or permitting the distribution of the first doses.
Monitors patient’s total drug therapy checking for drug interactions, allergies, incompatibilities and appropriateness of therapy and makes recommendations directly to the prescribing physician as necessary, taking into consideration the special needs and behaviors of the specific age of the patient.
Documents all medication clarification/interventions in the patient’s medical record and/or pharmacy profile as appropriate.  Reports all clarifications/interventions via the pharmacy productivity reporting program.  Documentation in the patient’s medical record is according to established policies and procedures.
Enters medication data into the pharmacy data processing system assuring that data is current and accurate and that patients receive medications in a timely manner.
Provides information to physicians and other hospital staff members of availability of new drugs, warnings on currently marketed drugs, incompatibilities of certain drugs and contraindications of drugs and other pharmaceuticals.
Participates in the Pharmacy’s pharmacokinetic dosing service in consultation with the clinical pharmacist on call.
Provides Pharmacy services to the operating room, PACU, and same day surgery unit via the Pharmacy Operating Room Satellite.
Conducts and/or participates in inservice education programs as necessary.
Conducts inpatient and outpatient medication counseling and education.
Compounds various pharmaceuticals, prepares and monitors sterile parenteral medications and other specialty items using standard formulas or as specified by the physician.
Enters orders into the Pharmacy data processing system and fills prescriptions for both outpatients and employees, dispensing in accordance with applicable laws/regulations and established procedures; updates outpatient medication profiles on all patients.
Participates in the established process for assessing, managing, and reporting medication errors.
Maintains the security of controlled substances and a perpetual inventory of Schedule II controlled substances.
Monitors and directs the work activities of the certified and non-certified Pharmacy Technicians; checks and approves all work involving the handling of drugs and drug orders before dispensing to patients and hospital departments for accuracy.
Monitors the inventory of all drugs, requisitioning drug supplies as necessary.
Actively participates in the departmental continuous quality improvement program and adherence to medication-use policies.
Complies in all activities within accreditation, legal, regulatory and safety requirements.
Assists with orientation and instruction of University of Illinois College of Pharmacy
students and South Suburban College Pharmacy Technician students.

This position is full-time, 80 hours per pay period, evening shift, 2:30 p.m. to 11:00 p.m. with every 3rd weekend (Saturday and Sunday).
