Overview:
We are looking for Registered Nurses for Home Health Care. Responsible for patient care in home settings, conducting assessments, coordinating with physicians, and delivering skilled nursing care.
Duties:
Perform home care assessments.
Develop individualized care plans.
Implement the Plans of Treatment
Provide skilled nursing care.
Communicate with physicians and interdisciplinary team.
Document patient care accurately.
Participate in quality improvement initiatives.
Adhere to infection control protocols.
Requirements:
1+ year of professional experience as an RN (Home Health experience preferred)
Active RN License in the State of PA
Associate degree (or higher) in Nursing
BLS and CPR Certification (if you don't have this - we will help you get it!)
Valid Driver's License, Reliable Mode of Transportation, and Auto Insurance
