Job Description
The Community Health Worker (CHW) with the Health Start program assists and supports in obtaining health-related services for clients of the NATIVE HEALTH, Prenatal Clinic and WIC. The CHW is a liaison between the Department of Health Services, the health care provider, and the Phoenix Metropolitan American Indian Community. The position aims to increase early and adequate prenatal, postpartum, and inter-conception care for women in Maricopa County, as well as promote optimal development of their children from birth through age two.
KEY ACCOUNTABILITIES:
Deliver services primarily through prescheduled visits to the client’s home, office, or public setting and scheduled health-related classes.
Identify, screen, and enroll eligible, pregnant, and postpartum women, caregivers, and their children in the program in accordance with the Arizona MIECHV policy and procedure manual.
Participates in training consistent with program objectives to provide education to benefit enrolled participants.
Accurately documents and keeps records of all visits and case management services provided to families.
Supports the organization as needed.
