Hours: 36 hours per week

The Role: As an Independent Living Coordinator within our Complex Care Services Review Team, you will hold a pivotal role in assisting adults with complex health and social care needs to maintain independence within their homes. Your duties will encompass:

Assessing individual requirements
Orchestrating support services
Advocating for optimal outcomes for our service usersMoreover, you will be instrumental in:

Identifying and implementing assistive technologies to enhance autonomy and cultivate an enhanced quality of life.About You:

Possess GCSE Maths and English or equivalent qualifications
Hold a Level 3 Diploma/NVQ in a relevant field
Demonstrate proficiency in care coordination principles and the Trusted Assessor role
Display comprehensive knowledge of disabilities and conditions impacting individuals' autonomy
Capable of collaborative work within a multi-disciplinary team
Exhibit strong communication abilities and adeptness in engaging with diverse communities
Bring experience in assisting physically disabled or elderly individuals, preferably within a Local Authority context
Show proficiency in utilising computer databases (e.g., Mosaic)
Possess financial acumen to facilitate individuals with Direct PaymentsWhat We Offer:
